Acceptance of Services at Your Own Discretion:
By checking the child in for service, the accompanying parent/legal guardian or authorized adult gives permission for the child to receive a complimentary basic haircut, hair-trimming service, or mini facial at this event. I understand that these are limited event services and do not include a full salon consultation, medical treatment, or professional skin evaluation. Results may vary.

I have disclosed any relevant allergies, sensitivities, scalp or skin conditions, active irritation, lice exposure, medications, or other concerns that may affect the service. I understand that the service may be modified, declined, or stopped if the child experiences discomfort, irritation, or if event staff determine that the service should not proceed.

I release the event organizers, venue, volunteers, and service providers from claims arising from ordinary participation, except where prohibited by law.
